There's a wait, but not a long one, and only the first person gets it. Waking takes a few seconds to a few tens of seconds depending on how big your app is and whether it connects to a database. Everyone after that gets normal speed. If your app needs to stay instantly available, you can turn sleeping off — that tier never sleeps, at the cost of holding resources permanently. Sleeping is on by default because for almost every freshly built app, "free to leave up" is worth more than "half a second faster for the first visitor".
